TO RENEW YOUR CSAM MEMBERSHIP You can now use the shortened Renewal Form and submit one of three ways:

FOR FIRST TIME MEMBERSHIP

Please fill out the membership application form

1.    You will be asked for your contact information, history, areas of interest and to provide the name of one referree.
2.    Membership dues can be paid for 1, 3 or 5 year allotments
3.    There is also the option to pay for membership in the Intrnational Society of Addiction Medicine [ISAM]
4.    Once you have filled out the form, please fax to 1-403 944-2056   Attention: CSAM

FULL Membership is open to applicants who hold a Medical Degree or to PhD scientists with demonstrable commitment to the objectives of the Society.  Membership is also available to medical students, interns, and residents and retirees. 

ASSOCIATE Membership is open to those persons with a demonstrable commitment to the objectives of the Corporation and the goals of the Society. Applicants for admission as Associate members will have received the sponsorship of an active Full Member of CSAM. Associate members will have all the rights and privileges of membership except the rights to elect and/or become a Board Member and to vote at meetings of members.

HONORARY Membership is an honour bestowed on members of the Society who have demonstrated distinguished service to the Society and the field of Addiction Medicine.
